Weighing Up The Alternatives?

While most non surgical procedures are not a permanent solution they have improved significantly in recent years. These procedures can delay the need for surgery and also give you a look at the effect obtainable from more refined permanent surgery at a later time.

Procedure that uses injectable gel fillers, and neurotoxins can provide a gently lift in facial features and reduce wrinkles can last for up to two years.

As with any treatment, always do your research thoroughly discounted rates or cosmetic enhancement holiday destinations can end up with some very unpleasant health issues and outcomes.

 

Tell Me About Botox

Botox is a trade name (and there are others such as Dysport) of a compound that inactivates muscles. Interestingly it’s a toxin, a neurotoxin to be exact which means it acts on blocking the action of nerve on muscles. Don’t worry, the doses used around the world is very very safe – a typical bottle of Botox contains 100 units of Botox, you need in excess of 5000 units for it to be harmful!

By stopping the action of the muscles it smooths out wrinkles and this effect lasts for few months. Typical areas treated are frown lines, the crow’s feet around the eyes, the horizontal lines on the forehead, neck wrinkles and gummy smiles. This is done with smallest of needles in the office and depending on the areas to be treated it can takes 30 minutes to an hour. Botox is also used in the reconstructive field outside of aesthetic medicine- it is used to treat excessive sweating in the armpits (for which there is support from Medicare), migraines and spasticity to relieve tight muscle activities to name is few.

Who is Botox NOT suitable for?

Some examples of contraindications are as follows.

  • If you’re pregnant.
  • If you have conditions called neuromuscular disorders (one example being myasthenia gravis) which means inherent problems in the nerve acting on the muscles to begin with. You cannot have botox that further weakens muscles.
  • If you have allergy or hypersensitivity to Botox.
  • Drooping of the eyelid (called ptosis)
  • Those with breathing and speech problems.
  • Those with unrealistic expectation about what it can do for them and who refuse to acknowledge potential side effects.

What are some side effects of Botox?

  • Bruising– can happen and lasts less than 2 weeks in most cases. This is why gentle pressure is applied when injecting to minimise the risk of bruising.
  • Infection– in theory it’s possible but rarely a problem.
  • Droopy eyelid– because Botox works by stopping muscles it can unintentionally stop the muscles that work to open the eyes wider. This is why its is important to employ correct technique, to not massage the areas afterwards and stay out of sauna or strenuous activities that may lead to your blood vessels opening up. We want Botox to stay in the areas we injected and minimise the risk of it travelling to unintended places. Although rare if this happens it can be treated with some eye drops.
What About Fillers?

Fillers, along with botox is one of the most commonly performed non surgical procedures around the world. It’s effects lasts months and it can be used to bring about subtle changes either on its own or as a combination of other procedures. The common brands such as Juvederm and Restylane have different names within them- it basically comes down to different size of the particles within the fillers.

Fillers these days are made of substance called hyaluronic acid, almost gel-like substance that has some strength to them. These fillers are then injected into very specific areas to restore volume to replace what is lost with aging process.

Some areas like lips will most often need a filler with smaller particles because it is more delicate and closer to the surface so you can ‘feather’ more easily. For volume in the cheek for instance however you need a bigger particles because the filler is placed in a deeper plane. This is the reason for different brand names even within each trading names.

The benefits of fillers you already know- it is well publicised, some times excessively so. Our job here to to provide a balanced view, to inform and to educate to make sure you are undertaking every cosmetic procedures fully mindful of potential risks so that you can make an informed decision.

What Can Go Wrong With Fillers
  • Failure to achieve a desired result– commonly this is due to uneven surface or lumpiness. Fillers can some times result in reactions that result in prolonged inflammatory process because your body “over reacts” to the injected filler. This can also be due to infection– fillers are foreign body that is introduced into the body and in great majority of the time cause no problem in the body, but if bugs get onto the foreign substance it can lead to prolonged reactions. This can lead to lumps that may need to be dissolved away with counter active substance called hyalase.
  • Blockage of small vessels leading to tissue problems- Again, filler is a volume creating gel-like substance and is placed in the body using needles. In rare cases it can inadvertently be injected into vessels and this leads to blockage of that vessel. This risk is rare but if this happens it can lead to stopping the blood flow to that vessel it was injected into. Great majority of the time this leads to minor wound problems in the skin that improves over time but there are documented cases around the world, particularly for fillers around the eyes that has lead to blocking of the artery to the eye causing blindness. This is the reason that fillers around the eyes are undertaken only by those that are extremely proficient and well versed in this procedure.
  • Hypersensitivity reaction to the filler- again this is relatively rare.
Non-Surgical Procedure For a Double Chin

Double Chin Reduction  This is done with a fat dissolving injection. It is done in the office much like botox and fillers.  A series of injections under the chin with deoxycholic acid  permanently dissolves this fat. Usually more than one treatment is needed so they are spaced out over few weeks each visit taking between thirty minuts to an hour.

What Can It Do For Me?

Prior to this injection being released the only way to address it effectively was with surgery. In some patients liposuction in the operating theater is still the most effective way to address fat under the neck. But for those with only moderate amount of fat under the chin this injection has made an office based procedure possible. The first things to know about this injection is the swelling under the chin which peaks at about 2 days. This swelling then gradually settles over a week and you are seen again in a few weeks to see the result to repeat the subsequent treatment. It costs between $1000-$1500 per treatment and progress is discussed with you following each treatment to see whether you would like the subsequent treatments- most have 2-3 treatment sessions to see the final result.

What Side Effects Can Occur From Double Chin Reduction Injections?

The following side effects range from rare to extremely rare.

  • Numbness under the chin- this effect is usually temporary and settles down over days.
  • Firmness under the chin- can happen in less than 20% of patients and last for up to 2 months.
  • Infection- rare.
  • Allergic reaction- rare.
Laser rejuvenation, Dermabrasion, Chemical Peels

Once upon a time people reached certain age and those that wanted to look the way they felt they went to see a plastic surgeon and got a facelift. These days though, people still do get a facelift, but later and some times never. The reason for this is because of the huge explosion in non surgical modalities such as laser and others on this page that allows one to prolong, protect and rejuvenate for much, much longer. Laser is one of the ways that allow the minimum down time and regular “maintenance” treatment and Dr Kim works closely with specialist dermatologists whose only concern is skin. Regular “up keep” if you like, of skin care products and non surgical treatments will go a long way in looking the way you feel without an immediate need for surgery.

The principle of laser and other non surgical treatments such a dermabrasion and chemical peels are similar. It is stimulation of deeper layer of skin called dermis to produce more collagen, the building block of our skin and one that gives us more youthful appearance. Baby skin has a ton of collagen and different type of collagen as opposed to an older person and the aim of these non surgical treatment is replicate this. Although the results may not be as dramatic as surgery these non surgical treatments are excellent in subtle changes that lead to prolonged youthful appearance over time.

Laser

Laser achieves this by creating tiny columns in the skin in a set pattern. If you look at treated skin under a microscope you will see tiny columns through the top layer of skin into dermis which then leads to stimulation of collagen. Each patient need to be assessed to see whether you are suitable as some darker skinned individuals need different treatments because the risks can be change in the skin tone.

Chemical Peels

Chemical peel is another way to stimulate collagen production in dermis. Chemical substance are applied to skin which causes a light sunburn like reaction to the top layer of skin, the epidermis. (Please note though that sun burn itself is absolutely bad for skin- sun light is bad because it causes skin cancer and it causes ageing so do not mistake sun burn for any rejuvenation, it will cause the opposite!!!)

Dermabrasion and Micro-needling

Again, the idea is similar to laser and chemical peels- it physically removes top thin layer of skin so that the deeper skin layer, dermis can be stimulated to form more collagen. Like most of these non surgical treatments suitability for them should be assessed by specialists whose prime interest is in aesthetics.

  • Risk of scarring– we unfortunately see this resulting from untrained and unregulated “clinics” that cause uncontrolled injury to the skin. These treatments need to be absolutely undertaken by specialist dermatologists and plastic surgeons whose prime interest is of the skin with years of specialist training to understand the fine nuances of different treatments available in an accredited facility that perform these procedures on a regular basis. Unfortunately many non surgical clinics are almost totally unregulated in Australia.
